By scheduling and/or paying for any service/class offered through DivineRadiantLiving.com or with Danielle Kitzes and/or communicating with Danielle Kitzes in any fashion, you are attesting that you have read this legal disclaimer, these terms of service and these disclosures as well as the attached waiver.
Further, you attest that you understand and willingly enter into, agree with and accept the following:
1. You are 18 years of age or older.
2. 100% accuracy is not guaranteed.
3. Any information, communication and/or consultations with Danielle Kitzes and/or DivineRadiantLiving.com website are for educational, spiritual and entertainment purposes ONLY.
4. No healing, reading, class or service offered by Danielle Kitzes and/or DivineRadiantLiving.com is intended to, nor should it take the place of professional services including but not limited to: medical, legal, financial, business and/or psychological.
5. Danielle Kitzes and DivineRadiantLiving.com accept no liability and/or responsibility for any actions and/or decisions any client or student chooses to take or make based on his/her session/class with Danielle Kitzes and/or DivineRadiantLiving.com
6. You agree to absolutely indemnify and hold harmless Danielle Kitzes and DivineRadiantLiving.com, their heirs, relatives, agents or assignees in perpetuity and throughout all known and unknown universes.
7. Sessions are scheduled for and paid in advance. If you cancel 48 hours in advance you can opt for a full refund. If canceled less than 24 hours in advance, the payment will not be refunded but can be credited towards a future service/session.
8. All services/classes offered by Danielle Kitzes and/or DivineRadiantLiving.com are for entertainment purposes only.
9. You have had the chance and taken the time to ask any and all questions, you fully understand and of your own free will, choose to enter into this legal and binding disclaimer and terms of service agreement.
· Medical Disclosure - Please read
o I understand that Reiki practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional.
o I understand that Reiki does not take the place of medical care. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have.
· Reiki Waiver - Please read
o I understand that Reiki is a simple, gentle, hands-on energy technique that is used for stress reduction and relaxation.
o I understand that Reiki can complement any medical or psychological care I may be receiving.
o I also understand that the body has the ability to heal itself and to do so, complete relaxation is often beneficial.
o I acknowledge that long term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself.
· Energy Work / Reading Disclosure - Please read
o I understand all energetic/spiritual/intuitive/medium readings of any type are for entertainment purposes only.
o I understand the Practitioner is not a medical or legal advisor nor a licensed therapist or medical professional.
o I understand and readings in no way constitute and/or replace legal, financial, medical or any other professional advice.
o I understand the Practitioner cannot diagnose conditions, nor prescribe or perform medical diagnosis or treatments.
· License Disclosure - Please read
o Practitioner is Reiki I & II level and a Yoga Teacher, and is not a licensed physician or licensed therapist. Treatment is alternative to healing arts services and what is provided is not licensed by the state
